Our interprofessional teams include health professionals, educators and support workers<br \/>\nrepresenting both traditional Indigenous and contemporary models of care and using wholistic health and community development concepts.<\/p>\n<p>The Clinic Nurse\/Care Coordinator is part of a client-centered professional team structure and reports to an Interprofessional Team Manager. The clinic nurse works with a variety of registered and non-registered<br \/>\nhealth care providers, both directly and under the facilitation of medical directives, to facilitate coordination of client care and support optimal functioning of the team to provide health care services.<\/p>\n<p>This position is part of the Ontario Nurses Association Bargaining Unit.<\/p>\n<p>NOTE: Traditional practices involving burning sacred medicines including tobacco, sweetgrass, sage and<br \/>\ncedar and other cultural protocols occur regularly within our work setting.<\/p>\n<p><strong>QUALIFICATIONS:<\/strong><\/p>\n<ul>\n<li>Current registration as a Registered Practical Nurse with the College of Nurses of Ontario<\/li>\n<li>Willingness to work to a full scope including performing delegated acts through medical directives<\/li>\n<li>Knowledge of provincial immunization schedule and experience with the administration of vaccines<br \/>\nan asset<\/li>\n<li>Training and experience in venipuncture procedures and wound care an asset;<\/li>\n<li>Experience and demonstrated ability to work within a wholistic care model; able to work<br \/>\nindependently and be an effective team member<\/li>\n<li>Experience working in an Indigenous organization and ability to understand and speak<br \/>\nAnishinaabemowin a definite asset<\/li>\n<li>Willingness and ability to travel<\/li>\n<li>Valid drivers\u2019 license and own vehicle required; copy of current drivers\u2019 abstract required<\/li>\n<li>Current criminal records check required<\/li>\n<li>Commitment to ongoing training and professional development relevant to the job<\/li>\n<li>Positive attitude and capacity to act as a healthy lifestyle role model<\/li>\n<li>Proof of immunization in compliance with policy requirements is mandatory<br \/>\nSalary: $30.32 &#8211; $38.24 per hour with excellent benefits, including HOOPP pension plan.
