If you are rostered with WNHAC for primary care, you do not need a referral to see our diabetes team; you can schedule by calling 1-888-MYWNHAC to make an appointment.
If your primary care provider is outside of WNHAC organization & they do not have diabetes services available at their clinic, your primary care provider would need to send a referral.
